Both games start each run with an offer from Neow. Slay the Spire 2 adds more Ancients at the start of acts 2 and 3.
StS1 Neow
Neow offers four choices if your last standard run with the same character reached floor 16 or won. Otherwise Neow offers two smaller choices.
The four choices always come one from each group:

The group 3 drawback is one of: lose 10% of your max HP, lose all gold, obtain a curse, or take damage equal to 30% of your current HP. Some pairs never appear together: remove 2 cards is not offered with the curse drawback, 250 gold is not offered with the lose-all-gold drawback, and 20% max HP is not offered with the max HP drawback.

Neow appears after you complete your first run. Darv appears only after you unlock him. Each Ancient offers three relics and heals you; at Ascension 2 the heal covers 80% of your missing HP instead of all of it.
StS2 Neow
In a normal run Neow offers two relics from the first list and one relic with a drawback from the second.
